With every vehicle has a TITLE being exported out of the USA, Export Power of Attorney is a required document.

 

At this time Florida State requires ORIGINAL export documentations along with original title.

 

Please print below form, and fill with your best knowledge. If you have missing information please contact us or just leave it blank.

 

The lower part of the page, require notary public signature and stamp.

 

If you are having problem with notary, please go to nearest American Embassy and they will do this for you.

 

Required documents:

- Original Title.    Assigned to shipper name.

- Original POA.    Signed by the shipper and stamped by notary

- Copy of shipper Passport or valid picture ID.

- Copy of cargo invoice,    only in some cases.
